Once you've decided the time is right to make some healthy lifestyle changes, book an appointment with Rita and you will receive a short questionnaire and intake form that asks you about your nutrition and health goals, your medical history, and your eating and exercise habits. Once you've completed the forms, we ask that you email them back to us. This allows me to get a head start on your nutrition assessment before meeting with you.
During this 1 hour visit, you will receive nutrition recommendations that outlines the dietary and/or lifestyle changes necessary to help you achieve your goals.
These visits are critical to your success - follow-up visits provide support, accountability and allow for ongoing nutrition education. Follow-up visits can be combined with other services depending on your goals.
Follow-up visits may be scheduled weekly, bi weekly or even monthly.
